would you place commas around the authors name?

Some grammar rules (and embarrassing mistakes!) transcend the uniqueness of different regions and style guides. This new International Grammar section by OnlineBookClub.org ultimately identifies those rules thus providing a simple, flexible rule-set, respecting the differences between regions and style guides. You can feel free to ask general questions about spelling and grammar. You can also provide example sentences for other members to proofread and inform you of any grammar mistakes.

which way would you write this sentence:

1.) Executive Hoodlum: Negotiating on the Corner of Main and Mean by John Costello is a memoir...

2.) Executive Hoodlum: Negotiating on the Corner of Main and mean, by John Costello, is a memoir...

I think both are acceptable, but I would pick the second one so that the sentence is more clear and structured.

Both ways are technically correct.

I would get around the problem by writing "John Costello's Executive Hoodlum . . . . is a memoir..." This produces a tighter sentence too.

I would preferably omit commas and use the first format.
