First movie that came to mind was Eragon. Granted, the first book of the Inheritance Cycle was kind of juvenile (Paolini got better at writing as the series went on) but the movie was awful, and it shouldn't have been. Jeremy Irons, Rachel Weisz and John Malkovich? But they changed the book too much, and it was awful. It also gets more awful points for ruining the chances for the rest of the series to get film adaptations.
I actually thought that The Fault In Our Stars was a good movie, even if it didn't compare to the book. As for Twilight, I thought the movie was better than the book. Although I suppose that depends on what you wanted to get out of the book.
In general, I tend to like movie adaptations of book in proportion to how much the writer was involved in the film.
Case in point - Stephen Chbosky's The Perks Of Being A Wallflower. I'm pretty sure he directed the movie, and it was stellar.
