So, there is a website called "lovebookonline.com" and it is basically made for exactly what I want to do. It was an ad on my Facebook today. (Well played Facebook.) It is about $35 (+shipping) for a paperback book and $40 (+shipping) for a hardback.

Blurb might do print books as well - it's been too long since I've researched them.

As far as format:
I recommend start with a poem, then a page of dialogue or description. Another page photo or illustration from your family or lives together. This depends on whether you want the book to be seen just among family or the world. You may not want your family photos looked at by everyone. Your choice.
