For fictional disturbing books, there are plenty including The Shining, Geralds Game (both by Stephen King), Hannibal by Thomas Harris, The Road by Cormac McCarthy and The God Delusion.
Plenty of disturbing books on the market but the quality varies wildly.

The Cement Garden by Ian McEwan also disturbed me - that book begins with a teenage brother and sister 'fiddling' with their younger sister. McEwan's short story collection 'First Love Last Rites' features several really weird tales as well.
Another book, The End of Alice, by A.M. Homes, (which I haven't read) I remember the tabloid press in the UK labelling 'evil' and saying it should be banned
